What Is Integrated Care? Why Southeast Treats the Whole Person

If you have ever left a doctor’s appointment with a referral to another office across town, only to wait weeks for a second intake, you already understand the problem that integrated care is designed to solve. Southeast Healthcare has built its programs around a simple idea: physical health, behavioral health, and daily-life needs are connected, and care works better when it addresses all of them together.

The idea behind whole person care

Since beginning operations in 1978, Southeast has grown into a provider of integrated healthcare across eight Ohio counties, combining primary and behavioral health services under one roof whenever possible. It is that a person managing diabetes and depression at the same time, for example, often does better when both conditions are addressed by a coordinated team rather than two separate systems that never talk to each other.

What this looks like in practice

A patient might come in for a routine visit with our Primary Care Services team and, during that same relationship, have access to our Behavioral Health Services programs, Dental Services, and our in-house pharmacy. Southeast also connects patients to Vocational Services and Homeless Services when housing, employment, or life stability are part of the picture. Care regardless of ability to pay

Southeast serves people of all ages, backgrounds, and circumstances, and accepts Medicaid, Medicare, and many private insurance plans. Patients are never turned away based on their ability to pay.
