Everyone plays a critical role in ensuring our clients receive the care they need, from Case Managers and Care Coordinators to Team Leads, Nurses, Providers, and Pharmacists. Each team member understands the importance of blood pressure control and works diligently to make it a reality for our clients.
This journey toward improved blood pressure outcomes began three years ago when Southeast Healthcare partnered with the American Heart Association (AHA) and the American Medical Association (AMA) on their “7 Simple Steps to Get an Accurate Blood Pressure Reading” program. As part of this initiative, our team, led by Diana Briggs and supported by Southeast’s Paul Schmidt, worked tirelessly to promote accurate blood pressure monitoring techniques. The program’s success earned Southeast a stipend, which was used to purchase blood pressure cuffs for several clients, further enhancing their ability to manage their health at home.
