Southeast Healthcare announces the opening of primary healthcare services under our Federally Qualified Healthcare Center (FQHC) designation for residents of Delaware County

Southeast Healthcare has been serving individuals with behavioral health and vocational services needs at the 824 Bowtown Road, Delaware Ohio location since 2017.

Southeast Healthcare has established a primary health center site in Delaware, Ohio, and provides integrated medical and behavioral health care to populations living within the city of Delaware and adjacent areas of Delaware County. Primary care services will be offered to individual and families with and without insurance, and patients may qualify for a sliding fee scale depending on monthly income. We are able to assist individuals with Medicaid enrollment and marketplace insurance applications. No one is turned away based on ability to pay. 

Primary healthcare services include, but are not limited to, preventive care and chronic disease treatment, annual exams, women’s health, asthma and COPD care, blood pressure screenings and treatment, diabetes care, family planning, immunizations and vaccinations, PrEP, specialist referrals, STI, HIV, and Hepatitis C screening and treatment, transgender care, Medication Assisted Treatment including Suboxone and Vivitrol, and dental care consultation and referral.

Southeast Healthcare will continue to provide a wide array of behavioral health care services as a contracted provider of the Delaware-Morrow Mental Health & Recovery Services Board.

Southeast Healthcare operates as a Federally Qualified Health Center (FQHC) and State of Ohio Certified Community Mental Health Center that delivers services to vulnerable and at-risk populations living in urban and rural portions of central and eastern/southeastern Ohio. Southeast Healthcare’s leadership identified Delaware County as a key geographical area with unmet health care needs among an often-hidden low-income populations living in a county growing in population and wealth.

The new site will initially employ a Family Nurse Practitioner, Care Manager, and an Licensed Practical Nurse with staffing ramping up as we build sufficient patient demand. Southeast Healthcare will fill and deliver pharmaceuticals for Delaware patients from its downtown Columbus 340B pharmacy and will work with other partners to ensure access to radiology, lab services, and continuity of care relative to hospitalizations, emergency care, and other referrals. 

Southeast Healthcare’s primary care is slated to open June 9, 2020 and will see patients on Tuesdays from 8:00am – 4:00pm and has the capacity to offer telemedicine appointments.
