Individualized Support for Meaningful Mental Health Recovery

Access to Wellness aims to help support community tenure, successful recovery, and positive long-term outcomes for individuals diagnosed with a severe mental illness.

The program does this by providing recovery supports- a type of assistance intended to help an eligible participant with mental health needs start or continue their recovery. Recovery supports are provided based on individual needs and clinical judgment, so each person may receive different support from the program.

Access to Wellness supports adult residents of Franklin County, regardless of whether they receive ongoing behavioral health services or where they receive them. Participants do not need to be patients of Southeast Healthcare or any other community treatment. However, referred individuals will need to meet eligibility requirements.

  • If you do receive services from a community behavioral health provider, please first speak to your treatment team about a referral.
  • If you do not receive behavioral health services or have additional questions, please reach out to AccessToWellness@southeastinc.com for more information.
