Southeast Programs & Services
Through our Primary Care Medical Home (PCMH), Southeast puts patients in the center of their healthcare. The PCMH model of care creates care teams that help patients understand all of the services they will receive today, or may need in the future - even if the services are not being provided in the primary care office.
Our PCMH practice supports patients in learning to manage and help to organize their own care. Recognizing that patients and families are core members of the care team, PCMH practices like Southeast work with patients to provide complete care, better access to care and more coordinated care for their patients.
Our Southeast staff is here to help with your healthcare needs. We have convenient hours and provide services at multiple locations throughout the state.
Southeast programs and services reach eight Ohio counties serving all patients and clients regardless of inability to pay and will not deny access to services. Discounts for services are available depending on your income and family size.
We also accept Medicaid, Medicare and many insurance options.
