Reaching Out, Building Hope: Expanding Services for the Unhoused in Delaware County

At Southeast Healthcare, we believe in meeting people where they are, especially in the midst of their most vulnerable moments.

With funding from a Department of Mental Health and Addiction Services (OhioMHAS) grant* and the support of the Delaware and Morrow County Mental Health and Recovery Services Board, we can provide critical outreach services directly to individuals in Delaware County who are experiencing unstable access to housing and/or those who are unhoused. 

We’re proud to announce a new initiative to better understand Delaware County’s population and help remove barriers to obtaining quality services.

Thanks to the grant allocation, we have created an outreach team of dedicated case managers who are going out into the Delaware community and providing linkage and support to those in need. Our outreach team is going out into the community and connecting with individuals anywhere support may be needed. They are also distributing outreach kits that include:

  • Hygiene kits with necessities such as soap, washcloths, deodorant, toothbrushes, and feminine hygiene products
  • Backpacks that include reusable water bottles and first-aid items
  • Food assistance and bus passes to help clients access services
  • Narcan and fentanyl test strips, along with education on their safe use

But this initiative is more than supplies; it’s about listening, building trust, and helping individuals take the first steps toward long-term care and housing stability.

Many individuals experiencing homelessness face barriers that make it difficult to access healthcare and support. Southeast Healthcare is working to remove those obstacles through this outreach program by offering education, referrals, and compassionate follow-up care.

Our case managers aim to build ongoing relationships, learn more about the population’s specific needs, and tailor services to meet them. Every interaction matters, whether it’s helping someone connect with mental health support, obtain substance use treatment, or simply find a place to feel safe for the night.

Together, we’re enabling more individuals to access the care and dignity they deserve.
